    1. The Calm Dog, LLC, will endeavor to create as safe an environment as possible for the training and boarding care of the Dog(s) identified above (the"Dog") and will offer only safe, sound and responsible training, care and post-boarding training instructions. Owner agrees, however, that all animals may exhibit unpredictable behaviors and that The Calm Dog, LLC, will not be liable for the actions of the dog that are outside the control of The Calm Dog, LLC. Owner agrees to allow random stool samples to insure the health and safety of all dogs. Owner agrees that the recommendation by The Calm Dog, LLC, of any product or service is not a guarantee of performance or Owner's satisfaction with that product or service.

    2. The Calm Dog, LLC, agrees to indemnify and hold Owner harmless from and against any claims of the injury, expense, costs, loss or damages caused by the gross negligence or will misconduct of the Calm Dog, LLC, training while the Dog is in the sole custody, care and control of The Calm Dog, LLC. Owner acknowledges that The Calm Dog, LLC has no control over the Dog when the Dog is not in the exclusive custody, care and control of The Calm Dog, LLC and Owner agrees to indemnify and hold The Calm Dog, LLC harmless from and against any and all claims of injury, expense, costs, loss or damages caused by the actions of the Dog when the Dog is not in the sole custody, care and control of The Calm Dog, LLC

    3. Owner acknowledges and agrees that owning any animal, including the Dog, has inherent risks, including but not limited to the risk of dog bites to the Owner.

Owner fully understands the tools and techniques that are used to train and modify the behavior of their dog and agrees to the tools and techniques which include the Prong Collar (Herm Sprenger, fitted), Bark Collar, Dominant Dog Collar, Remote Training Collar (also known as an E-Collar), Slip Lead, Pet Convincer, Muzzles and the Flat Buckle Collar.

    5. Payment Policy: Payment Due in Full at time of dropping dog off for training. A deposit of $350 per dog is required to reserve your training slot. Payment must be made in full and received by The Calm Dog, LLC, prior to the commencement of training. If writing a check, the check must be made out to "Kirsten Bailey," NOT to The Calm Dog, LLC.
    Cancellation Policy: If the client chooses to not continue training after receiving approximately 50% of services, a refund will not be issued.
    If the Trainer feels that the Client will not listen to training advice or follow instructions, The Calm Dog, LLC, can, at our sole discretion, choose to cancel the training contract and issue a partial refund. By signing below, you agree to the terms outlined above in the General Policy & Pay and Cancellation Policy (required). For Board & Train, if the Client must cancel, a refund will be issued minus the $350 reservation fee.

    I hereby assign and grant to The Calm Dog, LLC, the right and permission to use, reproduce, distribute and publish the photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) made of me and/or my dog(s) at any time during the training and/or boarding of my dog(s) by The Calm Dog, LLC, and I hereby release The Calm Dog, LLC, from any and all liability resulting from such use, reproduction, distribution and publication.

    I hereby authorize the reproduction, sale, distribution, copyright, exhibit, broadcast and/or electronic storage of any and all such photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) without limitation at the discretion of The Calm Dog, LLC and I specifically waive any right to any compensation I may have for any of the foregoing.     Please note: Board & Train dogs will be picked up and dropped off by the client between 10:00 a.m. and 11:00 a.m. unless other arrangements have been made in advance. At time of Pickup, we will work with you to demonstrate training protocols, observe your understanding of training protocols and answer questions. We will go over in detail all that your dog has learned and work with you on how to implement the training into your daily life.

    Follow-up lessons are available to help ease the transition of your dog back into your home and to help you maintain the high standard for behavior we have instilled at our kennel.

    When you bring your dog in for Board & Train, please bring your Dog's food, one small bed, medication(s), including flea/tick and/or heart-worm medication (if they need to be administered while your dog is in our care).

    Do NOT bring any toys, water/food bowls, treats or bones!

    Drop-off Agreement
    When dropping off your Dog at our Board & Train Facility, please arrive between 10:00 a.m. and 11:00 a.m. We are a working kennel, so we work with the dogs on a tight schedule. Please be on time! Your dog will get onto the property safely and securely and have the best chance for success this way.

    Make sure that your dog is on a leash and be certain that the your Dog cannot slip out of the collar when going from your car to our property during the hand-off to staff. Be advised that your dog may be nervous, scared, anxious or excited and that any of those behaviors can lead to your dog escaping the collar and running off into the street or around the property. This is a safety issue for your dog, the staff and other client dogs on the property.

    When you arrive, please quietly hand your dog off to a staff member. Do NOT hug, kiss, bend down or use a high-pitched voice when coming or going. Please just hand off the leash to the staff member without expressing any emotion. If you do not follow these instructions, you may jeopardize your own safety as well as that of the staff member.

    The staff member will bring your dog back to the kennel and put him/her in the kennel safely.

    You can leave food and any medication(s) any any further instructions with the staff member. Please label your feed bag/container with accurate feeding instructions. Please label all medications with accurate administration instructions.

    We do not allow non-employees into the actual kennel as we have numerous dogs with serious behavioral issues and it will create an unsafe environment for all of the dogs as well as the staff, and most importantly, you!     Physical Activity Agreement:

    Your dog will be staying at a working kennel. As such, your dog will spend a good amount of time in the outdoors doing activities around natural objects such as dirt, grass, cement, trees, fencing, bushes, interacting with other dogs and humans and it is likely that your dog may get scratched, bumped or bruised. Your dog will likely get some wear and tear on them -- if you've ever been to summer camp, you know how it is. We take special care to ensure our facility is safe, secure and clean for all dogs and humans. All dogs sleep in kennels inside our home with a real family to best emulate your dog's experience in real life.

    As Owner, you acknowledge and understand that there are certain risks involved in dog training, boarding and care. You hereby release, indemnify and agree to hold harmless The Calm Dog, LLC, from any and all manner of damages, claims, loss, liabilities, costs or expenses, causes of actions or suits whatsoever in law or in equity (including, without limitation, attorney's fees and related costs) arising out of or related to the services provided by The Calm Dog, LLC.
